We appreciate the trust you place in Eye Associates for New Mexico each time you refer a patient to us. We have created this page for your convenience in contacting us, scheduling your patient’s appointments or downloading information that will make your referring process easy.

Submitting a Referral

Allows your practice to connect via a free dashboard directly with Eye Associates for New Mexico.  There you can securely submit referrals, attach any important documents, receive status updates and chat directly with our Referral Representatives!

Use Live Interactive Form

Fill out our online form to instantly request a referral for one of your patients.  Please attach any pertinent exam notes, imaging (if available) and a copy of the patient’s insurance cards using the Attachments option at the bottom of the form.

Submit as Online Form

Fax or Call our Provider Referral Representatives

If you do not wish to submit a referral online, please fax demographics, exam notes, imaging (if available) and copy of patient’s insurance cards to 1-866-784-3083, or you can call our Doctor2Doctor team to schedule your patient at:  505-345-4433 (ABQ) or 877-345-4433 (Toll Free)
