The Eye Associates for New Mexico Benefits Package Includes:
Medical Insurance
We offer a choice of three Presbyterian Healthcare Plans to qualified employees who are scheduled to work at least 20 hours per week. Coverage for eligible dependents is also available. Employees may enroll beginning on the first day of the month following their date of hire.
Dental Insurance
Delta Dental is our dental insurance provider, offering coverage for employees and their eligible dependents. Qualifying employees may enroll beginning on the first day of the month following their date of hire.
Vision-Related Benefits
Employees are offered a variety of vision-related benefits and discounts, such as eye exams and eyeglasses, as well as discounts on contact lenses, LASIK/PRK and selected oculoplastic procedures/treatments. Qualifying employees are eligible for these benefits on the 181st day of employment.
Flexible Spending Accounts (FSA)
Eye Associates provides regular employees hired to work at least 20 hours per week the option to enroll in a Healthcare Flexible Spending Account (FSA). FSA’s allow employees to make pre-tax contributions that can be used to reimburse eligible medical and/or dependent care expenses, while also lowering their taxable income.
Basic Life Insurance/AD&D
Eye Associates provides group term life insurance equal to one times the base annual salary (up to certain maximums) for regular employees scheduled to work at least 30 hours per week. The premiums for this plan are fully paid by Eye Associates. Qualifying employees are automatically enrolled effective the first of the month following 180 days of employment.
Supplemental Life Insurance
Qualifying employees have the option to purchase additional group life insurance for themselves and their dependents.
Disability Insurance
Eye Associates provides short-term/long-term disability insurance for regular employees hired to work at least 30 hours per week. Premiums for this plan are fully paid by Eye Associates. Qualifying employees are automatically enrolled the first of the month following 180 days of employment. This coverage is designed to provide partial income replacement for employees who are unable to work due to illness, injury or childbirth.
401(k) Retirement Plan
At Eye Associates, all employees have the opportunity to grow their savings for the future by contributing a portion of their pay to our qualified 401(k) Plan, either on a pre-tax or after-tax (Roth) basis. You can choose from a diverse range of investment options to help you build a plan that fits your goals.
